Refer a patient to the Great Circle Recovery clinic in Salem, Oregon

To refer a patient to our Salem clinic, please download the Salem referral form, complete it, and fax it to us at (503) 983-9899.

Patients will also need to sign a release of information (ROI) form. Please download the form and send it to us with the referral form, or ask your patient to bring it to his/her intake appointment.

PORTLAND clinic

Refer a patient to the Great Circle Recovery clinic in Portland, Oregon

To refer a patient to our Portland clinic, please download and complete the Portland referral form. The Portland clinic fax number is coming soon.
